Failed to Mount Userdata & Data

Hi.. I think this is noob problem. I can't flash few Custom ROM (like BrokenOS, SailfishOS port) due to this issue "failed to mount /dev/block/platform/msm_sdcc.1/by-name/userdata at /data". And untill now i couldn't understand why it's appear. And i think this problem that make my device always boot into Recovery.

Now I still can flash few ROM like official & unofficial builds of CM 13 & 12.1. Right now I use TWRP 3.0.0.

Is there anyone know why it would be? Thanks in advance :D
